Online Registration With The Practice
If you wish to register, complete both of the THREE forms at the bottom of this page. The GMS1 form, Summary Care Record consent form and the New Patient Questionnaire. These three forms need to be completed for every patients registering.
Submit the forms by:
- email to: email@example.com;
- fax 01480 862893;
- handing in the forms to reception; or
- post the forms to Great Staughton Surgery, 57 The Highway, Great Staughton, Cambs, PE19 5DA.
When you visit us for the first time, if you have not already done so, you will be asked to sign the form to confirm that the details are correct.
When registering, please fill out a New Patient Questionnaire below. Medical records take some time to arrive and this will help provide a medical background such as your medication, special accessible information such as using a hearing loop if you having a hearing impairment.
Note: by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure.
The FINAL form, called the Summary Care Record (SCR) 'consent form' must be completed and signed. A Summary Care Record is a summary of your GP medical records, and it tells other health care staff information about your medicines and any allergies. This may be useful as they can give you better care if you need health care away from your usual GP surgery:
- in an emergency
- when you're on holiday
- when your surgery is closed
- at out-patient clinics in hospitals
- when you visit a pharmacy
You can add more information by asking your GP, this could include:
- health problems like diabetes, dementia or any impairment such a sight or hearing problems
- details of your carer
- your treatment preferences
When you are treated away from your GP's surgery, health care staff can't see your GP medical records. Looking at your SCR can speed up care and make sure you're given the right medication and treatment. Staff will ask your permission to look at it (except in an emergency where you may be unconscious) and only staff with suitable levels of security clearance can access them, so your information is secure.
